目的

探讨地舒单抗注射液联合碳酸钙D3片对老年股骨转子间骨折(IFF)髓内针内固定术后患者恢复的影响。

方法

回顾性收集本院2022年2月至2025年2月行股骨近端防旋髓内针内固定术治疗的80例老年IFF患者病例资料,根据术后治疗方案的不同,将患者分为对照组(采用碳酸钙D3片治疗,38例)和观察组(采用地舒单抗注射液联合碳酸钙D3片治疗,42例),两组均治疗12个月。比较两组股骨颈和转子间的骨密度值、骨形成和吸收标志物[骨钙素(BGP)、Ⅰ型前胶原氨基端原肽(PINP)、碱性磷酸酶(BALP)和抗酒石酸酸性磷酸酶(TRACP-5b)]、炎症标志物[C反应蛋白(CRP)、白细胞介素-6(IL-6)、红细胞沉降率(ESR)]、髋关节功能优良率、并发症及不良反应发生率。

结果

80例患者均获得完整随访,随访率为100%。两组治疗6、12个月的股骨颈和转子间骨密度值均较治疗前逐渐升高,且治疗12个月高于治疗6个月(P<0.05);进一步组间比较显示,且观察组治疗6、12个月股骨颈和转子间骨密度值均高于对照组(P<0.05)。治疗12个月,两组血清BGP、PINP、BALP水平均升高,TRACP-5b水平均降低,其中观察组变化更明显(P<0.05)。治疗12个月,两组各项炎症指标水平均降低,且观察组更低(P<0.05)。治疗12个月,与对照组比较,观察组髋关节功能优良率明显升高、再发骨折率明显降低(P<0.05),两组骨折不愈合、骨折延迟愈合和内固定失效发生率及不良反应发生率比较无统计学差异无统计学意义(P>0.05)。

结论

地舒单抗注射液联合碳酸钙D3片治疗不仅能提高老年IFF患者的骨密度和骨代谢,还能减轻炎症反应、改善髋关节功能,降低再发骨折的风险。

Objective

To investigate the effect of denosumab injection combined with calcium carbonate D3 tablets on the recovery of elderly patients with intertrochanteric fracture of femur (IFF) after intramedullary nail fixation.

Methods

A retrospective collection of case data was conducted on 80 elderly IFF patients who underwent proximal femoral anti rotation intramedullary nail fixation surgery in the hospital from February 2022 to February 2025. According to different postoperative treatment plans, the patients were divided into a control group (treated with calcium carbonate D3 tablets, 38 cases) and an observation group (treated with denosumab injection combined with calcium carbonate D3 tablets, 42 cases). Both groups were treated for 12 months. The bone density values, bone formation and resorption markers [osteocalcin (BGP), type I procollagen N-terminal propeptide (PINP), alkaline phosphatase (BALP), and tartrate resistant acid phosphatase (TRACP-5b)], inflammatory markers [C-reactive protein (CRP), interleukin-6 (IL-6), erythrocyte sedimentation rate (ESR)], excellent hip joint function rate, and incidence of complications and adverse reactions were compared between two groups.

Results

The bone density values between the femoral neck and trochanter in both groups after 6 and 12 months of treatment gradually increased compared to before treatment, and were higher after 12 months of treatment than after 6 months of treatment (P<0.05). Further inter group comparison showed that the bone density values between the femoral neck and trochanter in the observation group were higher than those in the control group at 6 and 12 months of treatment (P<0.05). After 12 months of treatment, serum BGP, PINP, and BALP levels increased in both groups, while TRACP-5b levels decreased, with the observation group showing more significant changes (P<0.05). After 12 months of treatment, the levels of various inflammatory indicators in both groups decreased, and the observation group had even lower levels (P<0.05). After 12 months of treatment, compared with the control group, the observation group showed a significant increase in the rate of excellent hip joint function and a significant decrease in the rate of recurrent fractures (P<0.05). There was no significant difference in the incidence of nonunion, delayed union, and internal fixation failure between the two groups (P>0.05).

Conclusions

The combination of denosumab injection and calcium carbonate D3 tablets can not only improve bone density and metabolism in elderly patients with IFF, but also alleviate inflammatory reactions, improve hip joint function, and reduce the risk of recurrent fractures.
